The introduction of new process or practices within the RM scope or the amendment of existing to take account of new information, legislation/regulation or technology is a reality which has to be dealt as part of an MOC process and in methodical manner to ensure controls are maintained. If in response to an incident, near miss or even during a periodic procedural review there is reason to develop understanding of how a policy or procedure came into its present state the reviewer needs access to the history of changes. Tracking of such changes either by full document or section can be achieved by following a determined process and using the revision log.
